Cancer Care:
Cancer is a chronic disease with
intermittent acute episodes resulting in the need for care in various settings
including rehabilitation and long-term care. The multidisciplinary team,
familiar with the patient’s disease, treatment, and prognosis will integrate
all aspects of care and will assist the patient through all phases of an
exacerbation of disease or chronic disease. Hospice care: For families and
patients needing 24-hour per day nursing care and counseling for the
terminally ill, pain management and comfort remain paramount. Hospice
philosophy recognizes death as the final stage of life and helps patients to
continue an alert, pain-free life and to manage other symptoms so that their
last days may be without pain or discomfort surrounded by loved ones.
Palliative and End of Life Care:
The goal of the
Palliative Care Program is to help patients and residents achieve the
highest-possible quality of life throughout every stage of illness. Medical,
emotional, spiritual and social needs are at the forefront to help patients
and their families through the complex problems associated with serious, life
threatening illness.
Chronic Renal Failure:
The multidisciplinary team manages patients with renal failure maintaining
ongoing communication with outpatient hemodialysis center. An
interdisciplinary team of caregivers works closely to provide and care for
this patient population by maintaining laboratory values within range and
ongoing monitoring for disease related complications.
Diabetes Management:
For patients who need monitoring, teaching and treatment for diabetes mellitus
the multidisciplinary team focuses on prevention of complications of diabetes
mellitus and realizes that diabetic control is imperative. Our goal is to
achieve consistent excellence in the care of diabetes incorporating medical,
nursing and nutritional interventions on a comprehensive basis.
IV Antibiotic Therapy:
Patients are able to complete a course of complex intravenous therapy under
the supervision of Registered Nurses and Physicians. In many cases, patients
also require additional skilled nursing services such as isolation procedures,
and are at risk for other medical problems. On-going Infection Control
monitoring creates an environment in which complex IV therapies can be
administered safely and efficiently.
Cardiac/ Post Open Heart Surgery Care:
Skilled nursing care and medical treatments are provided to assist in the
prevention of complications and improve the rehabilitation of patients with
cardiac conditions. Patients post open-heart surgery patients are monitored
closely for incision healing and potential cardiac complications. The
multidisciplinary team provides a specially designed program for cardiac care
and work together to return the patient to the highest possible functional
capacity.Neurological Disease Care:The multidisciplinary team focus is to help
patients recover the mental and/or physical functioning affected by a
neurological condition. Patients who have suffered strokes, paralysis, loss of
memory, mobility or speech and the inability to eat receive Physical,
Occupational and Speech Therapies through a comprehensive individualized plan
of treatment.
Post Surgical Care: Post surgical care may require a combination of
intravenous medications, wound care, nutritional support, skilled nursing care
and physical therapy during the recovery period. The multidisciplinary team
focuses on incision healing, pain control and rehabilitation.
Vascular Disease:
The multidisciplinary team works closely to return the patient to the highest
possible functional capacity allowed by his/her vascular diagnosis. Patients
with cerebrovascular disease, peripheral arterial disease, venous disease,
lower limb arterial insufficiency, and non-healing leg ulcers are provided
care in an effort to increase independence and decrease complications of
vascular disease.
Wound Care:
The multidisciplinary team is dedicated to the care of patients with surgical,
pressure, and vascular wounds. This specialized team of professionals provides
comprehensive inpatient services for those who require expert care for skin
and wound healing. Components of the Wound Care/Skin Integrity Program include
specialty dressings, 24-hour attention and treatment, specialized airflow
mattresses, and tracking and documentation of outcomes
Enteral Nutrition:
Enternal nutrition involves the infusion of nutrients into a person's stomach
or intestine via tubes placed through the nose or a surgical opening into the
stomach. This feeding approach is most often used in patients who are unable
to eat independently or who have difficulty swallowing and help to ensure
optimal nutrition and prevention of weight loss and skin breakdown. Enteral
Nutrition helps to provide adequate nourishment to the patient, prolonging
life and enhancing both comfort and quality of life.
