Long-Term Home Health Care Program
The mission of our Lady of Consolation Long-Term Home Health Care is to support our patients, their families, caregivers, and one another, in providing compassionate care that will secure and maintain the optimal level of well-being of each individual in the home environment.
The Long-Term Home Health Care program, also known as the Lombardi Program or Nursing Home Without Walls Program, enables individuals with chronic illness or disabilities to receive nursing home-level of care in their own homes. Unlike managed care Medicaid programs, the person’s own physician remains in charge of the care.
The philosophy of this program is to provide each participant the individualized care that allows a person to remain in, or return to, the family environment. To qualify, one must have nursing home-level care needs and Medicaid.
Participants in the program may receive the following, if qualified:

- Personal Care, Home Health Aid, and Homemaker Services
- Respiratory Therapy
- Physical Therapy
- Occupational Therapy
- Speech Therapy
- Nutritional Counseling
- Medical Social Services
- Audiology Services
- Emergency Response Alarm System
- Laboratory Services
- Respite Care · Social Day Care and Transportation
- Housing Improvement
- Home Maintenance and Moving Assistance
- Medical Supplies and Equipment
- 24-hour Professional Telephone Consultation
