HANYS Pinnacle Award 2014

Our Lady of Consolation Nursing & Rehabilitative Care Center has earned the 2014 Pinnacle Award for Quality and Patient Safety from the Healthcare Association of New York State (HANYS). A member of Catholic Health Services (CHS), the 450-bed skilled nursing facility was awarded this prestigious recognition for its “Managing Behaviors Without Medication: Providing Care to Special Populations” program.

“CHS embraces the concept of high reliability for all of its clinical operations throughout the system. As part of this initiative, CHS has adopted a formalized Lean Six Sigma program, a continuous improvement strategy that addresses defects and wasted effort in a process,” commented CHS’s Senior Vice President for Medical Affairs and Chief Medical Officer Patrick M. O’Shaughnessy, DO, MBA, who accepted the award on behalf of Our Lady of Consolation at the HANYS 46th Annual Membership Conference in Albany. “Since 2009, CHS has used Lean Six Sigma to address processes such as improving stroke care, patient flow from the emergency department to a hospital bed, and improving patient satisfaction.”

The winning program sought to reduce the use of antipsychotic medication, with a special emphasis on residents with a dementia diagnosis. Although the skilled nursing facility’s performance in this area at the time the program began was already significantly better than the national benchmark of 21.5%, an overall further improvement of at least 23% was achieved. The interdisciplinary team included physicians, psychologists, psychiatrists, and staff from all levels. Interventions were customized to each resident’s needs, with the help of input from the resident and his or her family, with personal history, lifestyle, culture, and other variables factored in.

Accreditations

As a member of Catholic Health Services of Long Island, Our Lady of Consolation plays an integral role in the health care delivery system of the Diocese of Rockville Centre. Our Lady of Consolation is accredited by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO), Certified by the Centers for Medicare and Medicaid, and licensed by the New York State Department of Health.

Our Lady of Consolation received a 5-Star rating from the Centers for Medicare & Medicaid Services. This rating is based on State Survey Agencies that perform initial surveys and periodic resurveys of all providers. These surveys are conducted to ascertain whether a provider/supplier meets applicable requirements for participation in the Medicare and/or Medicaid programs, and to evaluate performance and effectiveness in rendering a safe and acceptable quality of care.
